Copy of birth certificate
Copy of parent/ guardian I.D.
Proof of address
Progress reports from previous school
Transfer letter from previous school
ALTERNATIVELY, COLLECT THE APPLICATION FORM PHYSICALLY AT OUR SCHOOL.
